Important supplemental insurance information
Supplemental insurance products may include accident, cancer, critical illness, disability, hospital indemnity, fixed-indemnity, voluntary, worksite, and other limited-benefit coverage.
These products are generally not substitutes for comprehensive major medical health insurance and may include waiting periods, elimination periods, pre-existing-condition limitations, exclusions, benefit schedules, maximums, offsets, or other restrictions.
Disability products may use different definitions of disability, income requirements, occupation classifications, elimination periods, benefit periods, and offsets.
Group coverage may be subject to employer eligibility, participation, contribution, enrollment, portability, continuation, and carrier requirements.
A quote, benefit illustration, request for information, or application does not create coverage or guarantee eligibility, approval, benefit payment, or policy issuance. Coverage is not effective until the carrier accepts the application or enrollment and confirms the effective date.
